SUBMISSION REQUIREMENTS
- No candidates with an AZ home address on their drivers license will be considered – please include a copy in submission
- Minimum 2 years of current Med Surg experience – REQUIRED
- Must be Tele ECG compliant, all of our units do Tele and they will have to pass an ECG exam – REQUIRED
- Certifications must be American Heart Association OR American Red Cross and active upon and INCLUDED IN SUBMISSION – REQUIRED
- No Nursys Hits – REQUIRED
- Banner will not approve RTO within the first two weeks or holidays. Can not exceed 7 days.
- ***PLEASE NOTE, BANNER HAS VERY SPECIFIC BACKGROUND REQUIREMENTS THAT MUST BE ADHERED AND ATTESTED TO. PLEASE LOOK AT THE COMPLIANCE REQUIREMENTS IMMEDIATELY***
- Travel experience – REQUIRED

- Resume with all facilities, facility size, beds, trauma level, and units listed
- Skills Checklist specific to the discipline (must be less than a year old)
- Active License (verification must be less than 30 days old)
- Certifications (must be active at time of submission)
